Show move/rename information in diff view

Now show information about file renames and moves in the diff view, both in
the change index and the file sections.

Can you include some screenshots of what this looks like?

Change Summary:: Added screenshot showing the diff view for a review that includes one renamed file and also one that has not been renamed. Files that have not been renamed are displayed as they were previously.
