This change adds a "Review" link into the list of actions on the lower right of the
request info block, which pops up the comments dialog with only the review tab
visible.  This should both serve to make it more obvious how to make general comments
on the request (bug 105), as well as make it possible to comment at all on review
requests which contain only binary files (but 197).

I've also started adding more verbose function comments in here.  Hopefully we can
expand the rest of the code similarly ;)
