This moves the logic for discovering extensions via entrypoint, loading

their classes, and setting up registrations out of _load_extensions()

and into a new _discover_extensions() method.
No logic has changed, just code movement. This work is being done in

preparation for upcoming work to add fine-grained reloading of

extensions.

Unit tests pass.
