Add new X-ReviewBoard-URL and X-ReviewRequest-URL e-mail headers.

This introduces two new headers that are sent along with e-mails. These are
useful for filtering when multiple Review Board servers are in use.

X-ReviewBoard-URL contains the URL to the Review Board server.

X-ReviewRequest-URL contains the URL to the particular review request under
discussion.

Sent an e-mail and saw the headers.
