This patch makes two improvements to notification sending:
- Notification emails can be disabled per-profile, in the account settings.
- Notification emails are not sent to the person who initiated the notification. That means that e.g. when someone adds a comment then he/she will not receive a notification email about his/her own actions.

Manually tested on company's internal Review Board installation. Both features seem to be working :-)
