Microsoft Teams Integration entry information for the Review Board website.

This review request has a Markdown file that contains the information that
should be used for the Integration entry on the RB website. The information
needs to be entered into a new Integration entry via the admin UI for the
RB website. There's also two .png files for the 1x and 2x icons that should
be used as the entry's logo.
Tested these out on my local RB website devserver.
