This transitions from old base.js files to index.ts to set up

namespaces for avatars, configForms, extensions, forms, and

integrations.
This will make it easier for us to begin transitioning to modules in

these bundles.

Been running Review Board with these changes, and haven't hit any

problems. Tested various aspects of the app, utilizing each of these

bundles.
Successfully built a package.
